  10. Physical examination report
    When your application is accepted by department, please do the examination within 3 months before.
    Also, please note that Physical examination report in English version in your serving hospital (including Hepatitis B [HBsAg, Anti-HBs antibody], Hepatitis C [Anti-HCV antibody] (within 6 months), Chest X-ray, Measles IgG, Rubella IgG, RPR/VDRL (within 3 months) before coming here. If the result demonstrates that you do not have anti-HBs antibody, you will need to be vaccinated against HBV and provide proof document to us when you come to Chang Guang Hospital. If the result demonstrates that you do not have enough Measles IgG or Rubella IgG antibodies, you will need to receive MMR vaccine and provide proof document to us when you come to Chang Guang Hospital.

  11. Travel Health Insurance (The insurance claim amount must be at least 1 million New Taiwan Dollars or more.)

  12. Identification photo/Passport photo

PLEASE READ CAREFULLY:

  1. It takes 12 weeks for processing the application at Chang Gung Memorial Hospital and the Health Department of Government in Taiwan. The documents need to be send 12 weeks before the date of your arriving. 

  2. Observers are not allowed to operate, you can only observe in the operation room.

  3. We offer free dormitories with quadruple occupancy if the application has been accepted.
